Broadway’s Kansas City Care Clinic has some additional resources for its intensive dietary management program. The CVS Health Foundation said it has awarded the clinic with a $35,000 grant to support the program, which targets patients with chronic conditions such as diabetes, hypertension and obesity.
Clinic staff will provide nutrition education and ongoing support to underserved populations making lifestyle changes, with the ultimate goal of empowering patients to make healthier choices to improve overall health and wellbeing, according to an announcement.
The CVS Health Foundation says it has an ongoing partnership with the National Association of Free & Charitable Clinics to improve access to care, improve care coordination, and help manage chronic conditions.
